Projects PROEMPOWER ProEmpower, a project funded by the European Union's "Horizon 2020" Research and Innovation program, has launched a call for tenders to identify an innovative solution for the self-management of Mellitus type 2 Diabetes that will make person-centered assistance . The procuring stations of ProEmpower are looking for a solution for the management of diabetes, which addresses the unmet needs in the current treatment of diabetes, such as the fragmentation of today's healthcare system, the ineffectiveness and dependence of pharmacotherapy, the late diagnosis of disease and increased frequency of serious complications. This will be achieved by providing personalized and multidisciplinary assistance to diabetic patients, making them responsible to create responsible actors in the care process. The care will be based on the person, guided by a personalized decision support system, which will make it easier to set the appropriate treatment goals, based on the specific weight of multiple factors. Self-management education will be a key component of the solution adopted by ProEmpower that will favor the creation of a peer support network for patients, in order to help them to maintain a healthier lifestyle. VIGOUR VIGOUR project, is a project funded by the Third Health Program, which will effectively support care authorities in progressing and strengthening health and care systems to provide sustainable models of integrated care that will facilitate the identification of good practices. The care authorities will be supported in focusing their ambitions on integrating care, operationalizing the implementation of good practices adequately aligned to any prevailing local circumstances and finally introducing these practices to at least one percent of the overall target population within the framework of scaling-up projects. The exchange of knowledge and mutual learning during this process will be reinforced by a twinning scheme. The solutions to be developed should improve the results in managing frailty in the elderly impacting on physical and psychosocial factors. The target group is frail and pre-fragile elderly with particular emphasis on those who live alone. ADCARE ADCARE is a pre commercial procurement for the development of an integrated model, supported by information technologies, aimed at increasing the compliance to the therapy of subjects over seventy from the time of hospital access, both in hospitalization and outpatient, in continuity with the follow- up. In particular, a centralized platform will be developed and tested that will interface with the information systems of individual companies, in order to harmonize the procedures for prescribing and monitoring therapies from the hospital to the home of the elderly patient. Models and tools for the management of new biomedical technologies in the complex health field This project is a Pre-Commercial Procurement, funded by the Italian Ministry of Education, University and Research. The project aims to define and validate a solution for effective management of the life cycle of new biomedical technologies in complex health organizations. The project will provide innovative solutions to support investment planning, technological survey, management and disposal of biomedical technologies, taking into account the fundamental objectives of cost-effectiveness, risk mitigation and improvement of clinical results. ADVANTAGE The objective of the project is to elaborate a common definition of frailty to be used in all Member States, as a basis for the common management, both at an individual and population level, of elderly people who are fragile or at risk of frailty. The project, funded by the Third Health Program of the European Commission, aims to identify the components of frailty and its common management, both at an individual and population level, of frail people. Campania is involved in collaboration with the Marche Region, the Italian project leader, with ProMis, and with Liguria, Piedmont and Emilia Romagna Reference Sites. In addition, on December 13th 2018 was lauched the "Face up to Frailty Campain". The campaign aims to: - Give visibility to the ADVANTAGE JA recommendations among health workforce, policy makers and managers and older people and their carers. - Have social society contributing to implement ADVANTAGE JA recommendations and have tangible results in everyday life, thus putting knowledge and evidence into practice. - Raise awareness of the impact of frailty for individuals, families, communities and systems. - Support people to share their stories about living with frailty. - Help people from all sectors to understand what they can do to prevent and manage frailty. - Build readiness for a decade of active ageing aligned with the WHO Strategy and action plan. Through the centralized management of WE4AHA, the coordination activities and the external communication of the European Commission will be developed. Furthermore, stakeholders will be involved to help develop and implement three Commission demographic change activities: Market Innovation (I2M), Blueprint for the digital transformation of health services, European Innovation Partnership on Healthy Aging and active. The project is funded by the European Commission Horizon 2020 Program. EASYDOM The objectives of the proposal are to identify the determinants of acute events in the over-65-year-old population with multimorbidity, to identify the optimal intervention in the pharmacological, nutritional and physical field, and to test, through a clinical study, the effectiveness of an integrated assistance intervention, with ICT support. The project was presented and funded by the Ministry of Health, as part of Finalized Research 2016. |
